DISC Children, Young People and Families SDU provide a range of specialist services which support people with complex needs within a variety of community settings. Our programmes provide advice, guidance and support to address the issues which affect vulnerable children, young people and adults and include a range of educational and vocational preparation programmes.
The 4real service is a reshaped service which will support children, young people and their families to address drug and alcohol use. Working across County Durham the service is a multi agency programme with representation from Health, Social Care and the Voluntary Sector.
4real has replaced XS as County Durham’s young people’s substance misuse service. This exciting new role will engage with young people across the county, through outreach work, to promote harm reduction and impart information, advice and guidance.
You will have a sound understanding of the issues affecting young people who misuse drugs and alcohol you will be committed to the ongoing assessment of service user needs and provision of support to enable young people to achieve their goals.
An appropriate qualification, sound understanding of substance misuse interventions in both support and educational settings, and an open and none judgmental understanding of the holistic and complex needs of young people will prove invaluable in this role.
