DISC Children, Young People and Families SDU provide a range of specialist services which support people with complex needs within a variety of community settings. Our programmes provide advice, guidance and support to address the issues which affect vulnerable children, young people and adults and include a range of educational and vocational preparation programmes.
The 4real service is a reshaped service which will support children, young people and their families to address drug and alcohol use. Working across County Durham the service is a multi agency programme with representation from Health, Social Care and the Voluntary Sector.
Working as part of this exciting new team you will engage with a range of partnership agencies across the county in order to maximise referrals, ensure continued development of pathways and protocols and provide help and support to ensure the needs of children and young people are met.
Your understanding of young people's drug services together with your knowledge of developing effective networks and partnerships will ensure success in this post, as you promote the new service and establish frameworks and protocols to ensure consistent and relevant referrals into the service.
An appropriate qualification, sound understanding of substance misuse interventions in both support and educational setting and the ability to engage with a range of people will prove invaluable in this role.
